Heart 2 Heart - DVT: Blockage of a different kind
Mandy has just entered the second trimester of her first pregnancy. Just over a month ago, she was admitted with a painful swollen left leg. It started quickly and by the end of the day, the whole leg was swollen.
Ismail suffered a severe stroke a few months back. He has been confined to a wheelchair ever since he got out of hospital. Barely a week after discharge, his legs started to swell up. Doctors initially thought that he had heart failure as stroke and heart disease go hand-in-hand.
Abraham visited his son in London two years ago. He was a reasonably fit man in his sixties and enjoyed his two weeks of British summer. The day after he arrived home, he started to notice mild ache in his right calf. The following day, the right leg started to swell and become painful.
Mandy, Ismail and Abraham (not their real names, of course), have something in common. They have deep vein thrombosis or DVT in short.
DVT is the formation of thrombosis or blood clot in a deep vein. I have written quite a bit on blockage of the arteries - coronary and peripheral arteries - and I thought it would be good to talk about blockage in a different part of the cardiovascular system for a change.
Briefly, the arterial system brings blood full of oxygen from the left side of heart to the rest of the body and the venous system brings blood low in oxygen back to the right side of the heart. So when there is a blockage of an artery, the muscle or body tissue will be devoid of blood supply and oxygen causing cell death and organ damage. In a coronary artery, the result is a heart attack, and if an artery to the brain is blocked, a stroke will ensue.
Blockage to a vein does not usually cause any major tissue damage as oxygen and nutrients still reach the target organs via intact arteries. However, as the passage of blood back to the heart is obstructed, there will be accumulation of body fluid in the affected organ, often in a swift manner causing pain and swelling - a classical manifestation of DVT.
DVT usually affects the arteries of the legs and the pelvis (iliac veins). Whilst pain and swelling of the leg is troublesome, it is not life-threatening. The danger is when the clot breaks off and travels through the vein and goes to the right side of the heart which then pumps it to the pulmonary (lung) arteries. It will be dislodged in a blood vessel which is smaller than the clot thereby blocking blood supply to a part of the lung causing pulmonary embolism (PE). If the clot is big, then a large section of the lung will be affected. PEis dangerous and is a recognised cause of illness and death worldwide.

It is usually there after I walk for a distance and is gone when I wake up in the morning. Do I have heart failure and do I need to go for any tests? Answer: Swelling of the feet is one of the signs and symptoms of heart failure.

For a year and a half, I was basically waking up at four in the morning every night in a cold sweat. That just about encapsulates the experience. It was emotional torture. How much debt did you have? I maxed out 11 credit cards.
